Open reduction internal fixation (ORIF) is a surgery to fix severely broken bones.
It is only used for serious fractures that can’t be treated with a cast or splint. These injuries are usually fractures that are displaced, unstable, or those that involve the joint.
“Open reduction” means a surgeon makes an incision to realign the bone. “Internal fixation” means the bones are held together with hardware like metal pins, plates, rods, or screws. After the bone heals, this hardware isn’t removed.

Wear and tear on joints are a natural part of ageing, so too are the inevitable aches and pains that come with it. Osteoarthritis is the most common type of arthritis, affecting more than 56.6% elderly population in India. The bones that make up your joint surfaces are covered with a protective layer of cartilage that absorbs shock and vibration and allows the bones to glide smoothly during motion. Arthritis causes the cartilage to gradually wear away, resulting in a painful rubbing of bone-on-bone.
All of your joints may be affected by osteoarthritis, although weight-bearing joints, such as the hip and knee, tend to be the most painful. Other joints such as the shoulder and elbow are also involved causing annoying pain and functional disability. People who cannot function day to day because of pain or having rest or night pains and have restricted mobility are good candidates for surgery.
The most common joint surgeries are performed on hips and knees. However, replacement surgery can be and is conducted on other joints as well:
Hip replacement can be total or hemi (half). A total hip replacement addresses both the acetabulum and the femoral head. Hemiarthroplasty only replaces the femoral head.
The knee is the largest and most complex joint connecting major limbs and bearing the total weight of the body. It is susceptible to multiple injuries and ailments. Knee surgery options depend on the portion that needs to be treated – from a torn ACL to partial or total knee replacement to microfracture surgery.
Shoulder joint replacement includes the deltopectoral approach to save the deltoid and the transdeltoid approach, at the glenoid. Reverse Total Shoulder Replacement is an option in severe cases.
The elbow bones are replaced with an artificial joint made from implants that attach to the bones in the arm. A metal and plastic hinge joins the implants together.
In wrist joint replacement surgery, the damaged parts of the wrist bones are removed and replaced with artificial components.
Total Ankle Arthroplasty (TAA) is a surgical procedure used by orthopedic foot and ankle specialist surgeons to treat ankles affected by severe arthritis.
A finger joint (PIP) and knuckle joint (MP) replacement involve removal of the damaged joint and replacement with an artificial implant. This replicates normal joint function. The specific goals are to relieve pain, restore joint stability and mobility.
Generally, ORIF is an urgent surgery. Your doctor might recommend ORIF if your bone:
ORIF may also help if the bone was previously re-aligned without an incision — known as closed reduction — but didn’t heal properly.
The surgery should help reduce pain and restore mobility by helping the bone heal in the right position.
Despite the increasing success rate of ORIF, recovery depends on your:
